  • What is AfyaRekod

  • What is the Universal Patient Portal.

    This is a mobile and web based health data platform that places the patients at the centre of the health care ecosystem.

  • How does a patient access AfyaRekod?

    This is a mobile and web based health data platform that places the patients at the centre of the health care ecosystem.

  • Can I register as an organization?

    Yes. If you have a health service organization you can register it on the platform. (Onboarding step/process for organizations). Please reach out to us in case you need further guidance.

  • What is an AR code?

    The unique code each user is given on registering with Afyarekod. Think of it as your Afya (Health) Identification number.

  • What are Afya tokens?

    These are rewards that you receive upon completion of a service or activity, as it is introduced.

  • Is my information safe?

    Yes, it is, only those you allow access will be able to access your information.

  • How do I share my information?

    Go to the share center under your profile, select my shares, select share data, and fill in the information for the person or doctor you would like to share your information with. They will be able to view this shared data from their Afyarekod platform.

  • I changed my phone number/phone. Can I still access my data?

    Patients Know Best is a secure online record of your health and care information that you can access anytime, anywhere. You can also choose to share it with your health and care professionals, your relatives or your carer givers.
    With Patients Know Best you have 24/7 secure, online access to your medical records using any computer, smartphone or tablet. You don’t need to upgrade your computer or install new software and you control who gets to access your records. You can invite your doctors, nurses, carers and relatives, both inside and outside the NHS.

  • What information is held about me?

    Health and care professionals currently only have access to information held about you by their own organisation. With Patients Know Best, you (and your healthcare professionals) can view all the information held about you from every organisation that looks after you, all in one place.

  • Did you know?

    Health and care organisations that look after you already hold your medical information on paper or other computer systems. Putting the information in Patients Know Best gives everyone involved in your care the opportunity to see and add to it in one single place. This can help ensure you get the best care and avoid unnecessary tests and treatment.

  • Is my information secure?

    The UPP and AfyaRekod platforms are secured by blockchain technologies and we abide by local data privacy policies and its unique because it also gives you, the patient, access to your information via any web-enabled device. Patient information is encrypted (or coded) so that only those people with consent to view your information can do so and you are able to revoke access. We encrypt data in both transit (i.e. when the information comes in and out) and storage. With each patient record uniquely encrypted (i.e. we convert the data into a code to prevent unauthorised access), only the people whom you give consent to can decrypt (or un-code) and access the record. This approach means your information is always private and very secure.

  • I don’t use the internet but can I access my account with a care giver?

    You can (i.e. a friend, family member or loved ones) to use the Universal Patient Portal. This is useful especially to people with dependents. if you need help or want someone else to have access to your account. You can also control how much access the carer has to your record.

  • Is it easy to use?

    Yes. Learning how to use a new online platform may seem overwhelming at first, but don’t worry – Universal Patient Portal is designed to be simple, straightforward and secure.

  • How can I find out if my healthcare provider is using AfyaRekod?

    AfyaRekod is used worldwide and may also be set up in your local hospital or GP’s practice too. If not, please tell us, or them.

  • Where is my data stored?

    AfyaRekod uses high-end technology built on Blockchain and AI. Data is securely stored in the cloud which is remote and secure

  • What can I store on AfyaRekod?

    All your health records including prescriptions, vaccinations, scans and doctor’s notes stored in personalized medical folders. Best part, you own it and you control who can have access to your records.

  • Why do I need to store health data on AfyaRekod?

    AfyaRekod prompts patients to become more engaged in their health and take positive action to improve it, through daily record keeping of their health status and engagement with doctors, health experts, and other health service providers.

  • How do you use my email, phone number and the first login information i.e sex, age, blood type and the like?

    AFYA REKOD LIMITED uses the collected data for various purposes:
    1. to provide and maintain our Service;
    2. to notify you about changes to our Service;
    3. to allow you to participate in interactive features of our Service when you choose to do so;
    4. to provide customer support;
    5. to gather analysis or valuable information so that we can improve our Service;
    6. to monitor the usage of our Service;
    7. to detect, prevent and address technical issues;
    8. to fulfil any other purpose for which you provide it;
    9. to carry out our obligations and enforce our rights arising from any contracts entered into between you and us, including for billing and collection;
    10. to provide you with notices about your account and/or subscription, including expiration and renewal notices, email-instructions, etc.;
